What is the Washington University Follow My Health patient portal?
A patient portal is a web-based pathway that gives existing patients access to a secure, convenient and free way to manage their personal health care information at Washington University School of Medicine. Most, but not all of the offices of Washington University physicians participate in the program. You can learn more by watching this video:
What can it do for you?

Is it secure?
The first priority is patient privacy and the safety of patient information. For that reason, we must verify your identity. If you see more than one Washington University physician, you only have to sign up once. All records are stored on and transmitted via HIPAA - secure servers.
How do I sign up?
Invitation email from physician office: After the staff verify your identification, they will send you an email invitation to the address you provided. If you do not get the email, check your junk or quarantine file. Your account will only become active after you receive an email invitation from your provider. In the email is a link to the Follow My Health portal. Keep the email open until you are completely registered. This link ties and transmits your medical health record to the new portal account.
Using your physician statement: Visit wupatientportal.wustl.edu and fill out the patient identification request form. Once your information is verified, you will receive an email invitation at the address you provided (please allow up to seven days). If you do not get the email, check your junk or quarantine file. Your account will only become active after you receive this email invitation. In the email is a link to the Follow My Health portal. Keep the email open until you are completely registered. This link ties and transmits your medical health record to the new portal account.
How do I register my username and password?
You have two ways to access your health information:
1) If you wish to establish a unique username and password for your patient portal account, click on Create an Account and select the Follow My Health icon in the green shield.
2) You may use an existing username and password from a Google, Facebook, Yahoo! or Windows Live account, click on Create an Account and select the icon of the account username and password you want to use.
Continue the registration process on the Follow My Health screen. You will be asked for an Invite Code which is the last four digits of your social security number. This security check is to protect the privacy of your health account.
Log in to the site: Once the registration is complete, you can go directly to your health record using any computer or mobile device. We suggest you bookmark: https://wuphysicians.followmyhealth.com
Remember, you must be registered first, or your medical records will not be available to you.
Can I get accounts for my children?
You may open a proxy account for your child or children. Each patient must be seen in person at the Washington University physician's office to have his or her identity verified. At that time, you will be given a form with a proxy registration code. Then, please use the link below

I take care of a dependent adult. Can I set up an account for him/her?
People who are legal guardians, have healthcare powers of attorney or are responsible for the health decisions for a dependent adult, may enroll him/her. Each patient must be seen in person at the Washington University physician's office to have his or her identity verified. At that time, you will be given a form with a proxy registration code.
