Patients can be self-referred or referred by their physicians.Initial visits can last from one to two and a half hours depending on the individual's medical problem. Please ask your referring physician to forward records to the division by fax or by mail.
Please remember to check with your insurance company to see if a referral is needed -- and don’t forget to bring your insurance card.
