Requesting copies of your health information

You cay get a copy of your medical record created by and/or maintained by your Washington University Physicians provider. All requests are processed by our Health Information Release Services. To assure a prompt response, please choose the location where you received your care:

If you received care at these Missouri or Illinois clinical locations, please contact them directly for a copy of your medical records.

Fees

There is no charge for records sent directly to another physician.

Washington University partners with CIOX Health, the nation’s largest provider of release of medical information services, to process and fulfill your request for a copy of your medical record. We may charge a reasonable fee for copying of requested health information.

Washington University Physicians will respond to your request for health information within 30 days of our receipt of your request. If, however, your health information is not readily accessible by Washington University Physicians or is maintained in an off-site storage location, we have an additional 30 days to respond to your request. If we require additional time, we will contact you to inform you of this time extension.

Right to amend your health information

If you believe there is an error in your records, you can request that we correct them. We will review the information, as requested, and either make the correction or let you know why we think our information is correct.

Hospitalization records

If you need hospital records from Barnes-Jewish Hospital or St. Louis Children’s Hospital, you will need to request them directly from the hospital.
