Organizing Your Medical Information

In order to perform an effective consultation, we ask that previous tests and relevant information be forwarded to our office prior to the initial visit. Although this seems a time-consuming task when urgency is important, it is the only effective way to thoroughly review your situation without duplicating tests and procedures and ultimately taking more time.

Patients and their families can be very instrumental in getting information to us as soon as possible. Some referring physician offices or hospitals require a specific Release of Information Form signed before sending medical records; patients may want to check to see if a release is required and has been signed.

Referring physician offices should arrange for the transfer of lab data, pathology reports and slides, X-rays, CT scans and any other pertinent records. Regular mail service can take up to a week, so to ensure safe delivery we recommend using a delivery service, such as Federal Express or UPS to send material to the office where the patient is to be seen.
